Check Scheduling or Missing checks
If you have questions about check scheduling or
missing checks, call the revenue hotline toll free at
1-866-855-0518 or use the additional hotline numbers that
may be printed on previous revenue checks.
Owner Address/Name Changes
Address changes must be reviewed in writing. Send your name,
old and new address, and signature. For name changes as a result of marriage,
also provide your SSN for verification of your marriage certificate. Send
