I understand that material delivery may be dependent on where
crews are working, and during peak seasons, there may be a delay in receipt
of a delivery.
I agree that Dominion assumes no liability for my
use of the material.
I understand that shredded plant material has no warranty
of any kind.
I understand that this material has not been treated for
termites.
I understand that shredded plant material is a mixture
of shredded bark, wood chips, twigs and leaves, and it is not landscape-quality
mulch comparable to commercial-grade material. I realize the material
is "green" and may be too acidic to be placed around
some plants without damaging them.
I agree to use this material in a manner that protects
the public and the environment. Accordingly, I will not allow children
or pets to eat the shredded plant material, and I will not place the
material near a storm drain, lake or other water body. In addition, I will
not pile the material without "turning" it to prevent
fires, especially during dry weather conditions.
I release Dominion and its contractors from any
liability or cleanup.
I agree to provide my address and telephone
number on the request form in order to receive a delivery of shredded plant
material.
I understand I will be contacted prior to delivery by
a Dominion representative or an authorized Dominion contractor if shredded
plant material is available, and I will designate a delivery location that
is accessible to delivery vehicles.
